If you had private health insurance at the time of your ambulance service, then this fee may be covered depending on the specific terms of your policy.
Please forward the original invoice to your health fund to endorse that you were a valid member on the date of service. Your fund will need to forward an endorsed copy of the invoice to: SharedServicesAmbulanceFinance@act.gov.au
Alternatively, we can do this on your behalf. Please fill out the below form with your Health Fund membership details so that we can verify your cover for the date of service with your fund. By providing your membership details, you authorise Shared Services and/or their appointed representatives to record and verify your Health Fund membership on your behalf for the service provided.
Please note that if your Health Fund does not cover the cost of the service and there is no other entitlement or exemption available to you, payment of the invoice in full is required.
